Narrative
During a delivery of 14# natural gasoline the outboard seal on pump #3 failed, causing a release. Tulsa pipeline control center received a float seal fail alarm and pump shutdown automatically. Mechanic had replaced seals on pump #3 on 8/13/2104. Investigation determined that during the replacement on 8/13/14 mechanic failed to properly tighten the seal retaining bolts. Between 8/13/2014 and the release on 8/25/14 the pump had approximately 16 hours of run time. Supervisor and mechanics reviewed the procedure to ensure it was effective and no changes were identified. Mechanic was retrained on procedure.
