Narrative
A property owner contacted the control center about an odor and dead vegetation. An investigation was initiated and a leaking pipeline was confirmed. Repair and cleanup operations were initiated. The cause for the failure is an original pipe manufacturing defect. The leak was due to a lamination in the seamless pipe that collected hydrogen and leaked due to due to a hydrogen blister and step-wise cracking.
