Narrative
At approximately 8:40 am on thursday, october 2, 2014, a contractor using a 4" auger to install traffic signs struck a 6" plastic natural gas main line which resulted in a fire damaging the truck and auger. The ny department of public service was notified at 10:18 am. There were no injuries or fatalities. The one-call center was notified of the excavation activity. Investigation into the incident shows locating practices were not sufficient. Paint marks were found on the pavement marking an abandoned 6" steel main line. There were no markings for the 6" plastic line, which was offset approximately 6 feet from the abandoned line.
