Narrative
On october 1, 2014 a 30" distribution main leaked resulting in a reportable incident. At approximately 9:26 p.M. On 10/1/2014, dominion east ohio received an odor complaint at 7373 sherman church rd. A technician was immediately dispatched to the area. At 9:48 p.M. The technician arrived on site, conducted a leak survey of the area and concluded that gas was not migrating and according to available data did not believe the leak to be on a dominion facility. The technician notified the fire chief of his findings. At approximately 11:55 p.M. Dominion was notified by the epa that oil was bubbling up from the ground and that they were picking up high natural gas readings. Another technician was immediately dispatched to the scene, arrived at 12:47 a.M. On 10/2/2014, and upon investigation determined that deo's 30" distribution line was leaking and called in for a construction crew. The technician continued to leak survey throughout the night. As additional dominion personnel arrived on site, the investigation continued and determined that the line was deeper than a dominion crew could handle, therefore a contracting crew was mobilized. At approximately 5:00 a.M. Dominion's environmental contractors arrived on site and began environmental cleanup activities. The area was evacuated at approximately 6:00 a.M. As measure of safety, before excavating could occur dominion decided to lower the pressure in the pipeline. At approximately 10:00 am the pressure was lowered from 155 psig to 60 psig. Once the pressure was stabilized dominion began excavating. At approximately 5:00 p.M. A clamp was installed and pressure restored. The section of pipe that leaked was cut out and sent to kiefner & associates for a 3rd party failure analysis. The results of the test concluded that the cause of the leak was a through-wall internal corrosion pit at the 6 o'clock position. Test results suggest that microbial-induced corrosion (mic) played a role in the pitting corrosion. Dominion replaced approximately 800 feet of the 30" high pressure distribution line that leaked and will replace additional portions of adjacent pipelines throughout 2015.
