Narrative
A multi-vehicle accident involving a fatality occurred when a large moving truck crossed the marked center lane line into oncoming traffic and collided with several vehicles, including an f-150 truck. The collision ultimately forced the f-150 truck off the road, causing it to strike and damage a residential gas meter. The onsite investigation revealed that the f-150 truck sustained significant damage from the collision, including severing the fuel lines, and caught fire. Upon being notified by the fire department that the gas meter had been damaged as a result of the vehicle accident, ong responded to the scene. Ong personnel found the natural gas to have been ignited and burning in the open air. The fire department allowed ong to turn off the gas supply by closing the valve on the damaged riser. The investigation revealed that the f-150 truck was forced into the natural gas service meter located in the utility right-of-way, causing the fitting threads to break and allowing gas to be released.
