Narrative
At approximately 1530 hours, a third-party dig-in occurred near somersville road and james donlon blvd. In antioch. An excavator struck a 4-inch plastic distribution main that caused an unintentional release of gas. A pg&e gas service representative (gsr) and repair crew arrived on scene at approximately 1554 hours. The USA ticket (no. 0171085) is valid and facilities were properly marked. The third-party operator was aware of the pipe location but did not exercise caution and proceeded to dig. No police, no fire department, nor major media were on scene. There were no injuries, no fatalities, and no ignition during the incident. The amount of gas released was less than 3 mmcf. The gas was shut in at 1835 hours by isolating the main, which resulted in 280 customers losing gas service. Pg&e reached out to all 280 customers by 0100 hours on june 21, 2014. By 1030 hours on june 21, 2014, service was restored for 190 customers. As of 1450 hours on june 21, 2014, service has been restored for an additional 60 customers. By june 24, 2014, the 30 remaining customers (who were out of town during the relighting efforts) had gas service restored. This incident was reported to the cpuc because of operator judgment and also considered significant due to the large number of customers that lost gas service. Pg&e recently confirmed that damages relating to this incident exceed $50,000. Pg&e notified the national response center on july 17, 2014.
