Narrative
On monday, june 2, 2014, three employees were assigned to work on the regulator station in the vault at nw tyler & nw grant. The work involved replacing a weighted lever regulator with a worker/monitor regulator set style of station. The existing regulator was removed and new regulators were installed. Once installed, leakage was discovered on the new regulator station pre-assembled piping. The new station regulators were removed to make repairs. While repairs were being made to the worker/monitor set, it was discovered that a fitting used for one of the control lines was leaking. A welder was requested and dispatched to the work site to repair the leaking fitting. Blind flanges were installed where the new worker/monitor set had been removed and two employees began making a repair to the leaking fitting. A weld cone was installed into the leaking threaded fitting to stop the leak prior to finalizing the repair. During the containment of the leakage, as additional work was being conducted, a small flash fire occurred, causing superficial burns to one of the employees completing the work in the vault. The injured employee was hospitalized overnight to have his injuries examined and to confirm that the burns were external and minor in nature. The following supplemental report changes were made to this incident report - gas distribution system. 6/22/2015 bh page 1, part a, line 10. The total gas lost was 67 cu. Ft. This equates to.067 mcf. Rounding to 2 decimals gives a lost gas quantity of .07 mcf. This value was corrected as part of this supplemental report. Page 2, part a, line 13b. The local time this pipeline/facility was restarted was august 28, 2014 at 13:00 hrs. This time was added to the supplemental report. Page 2, part c, line 4a. This installation was done in 08/1953. The material checkout documentation only lists the pipe diameter and wall thickness. No other specifications were listed so the seam type is unknown. This response remains unknown. Page 3, part d line 2f. We have shown the gas lost as .07 mcf. Our cost of gas at the time of the incident was $6.5132. This equates to a cost of $.45. Since whole dollars are required, the cost of gas lost was $1.00. Page 3, part e, lines 1-3. This was a distribution line operating at 12 ounces. Because the pressure must be shown as psig, the pressure would equate to .75 psig. No changes were made to these lines. Page 4, part g1. This supplemental report corrects the cause of this incident as incorrect operation, part g7. With this change made, the information formerly shown in part g1 has been removed. See part g7 for corrected information. Page 9, part g7, line 1. Other incorrect operation was selected. The incorrect operation was not following procedures. Page 9, part g7, line 2. The selection, failure to follow procedure was selected. Page 9, part g7, line 3. The category type selected was other maintenance. A regulator/full relief valve setting was being replaced with a worker/monitor regulator station and being removed from a vault.
