Narrative
On may 2, 2014 at approximately 11:30 am, while performing site work for the corning museum of glass expansion project, the contractor was attempting to remove a drainage catch basin in the vicinity of the 3" hpde service pipeline. The utility locations had been mapped by a third party early in the project. The line was mapped and marked in a straight line but in actuality the line takes an offset so it ended up being in the path of excavation. The line was punctured by a tooth of the excavator bucket. The rupture was approximately 2"x8". The site where work was taking place was approximately 100 ft from a shut off valve and a valve wrench was on site for precautionary measures. The job site was evacuated (approx. 95 construction workers) while the gas service was shut off. Corning natural gas (corning inc's o&m contractor) was called to assess the damage and make the repairs. The corning museum of glass' hot glass show and caf� were shut down as a result of the gas outage. The museum remained open for business throughout the outage. Repairs were made and gas service was restored at 15:45. Both the equipment operator and site superintendent were immediately drug and alcohol tested following the incident; both tests came back negative. Insufficient line locating was determined to be the root cause. There were no injuries or fatalities as a result of this incident
