Four winds construction llc damaged the 2-inch plastic natural gas service line while directional boring in a conduit for fiber optic cable. Four winds did not request a locate for this project. Missouri state law requires the excavator to request the locate for the planned excavation activity. The locate was called by mastec construction (#140301962) but they did not name four winds construction llc as the excavator. Four winds construction did not attempt to identify the properly placed locate marks after they had been covered by snow. Furthermore, four winds did not pot-hole the service line that was damaged. The time and date stated in part a, 17a. Of this form is the time the fire department reported to laclede that an explosion had occurred. The report was verified and at 13:28 hours on february 5, 2014, laclede became aware that the incident met the reporting criteria of cfr part 191.3. Laclede reported the incident to the national response center at 14:26 hours on february 5, 2014.
