Narrative
During a scheduled maintenance activity a technician noticed a nearby pump leaking. This pump was not included in the scheduled maintenance. The 1/2" nipple on the pump had a 1/2" ball valve and a gauge screwed into the top. When the technician attempted to tighten the leaking apparatus to correct the leak the nipple broke at the threads. A total of about 1.5 barrels were released. Equipment was replaced, put back into service ,and contaminated soil was removed.
