Damage was caused by third partry contractor installing signal lines for the town of smithfield, ri. Site tech, the third party contractor, was using a vermeer to excavate and punctured our 6" main. The root casue of this damage was due to the contractor's negligence, in not complying with proper dig safe procedures.

The incident was caused by a third party damaging the company's gas main and the controllers were communicating with the company's gas dispatch group about what information the scada system was providing to them about the incident as the incident was unfolding. The communications between the two groups, each of which had individual pieces of information presented to them by two different sources, provided the complete picture of the incident and it's affect on the gas system in that area.
