Narrative
On march 7, 2012 at 09:55 a leak survey crew reported a t-1 leak at bradhurst avenue and stevens avenue, mt. Pleasant, along the route of the transmission main at this location as part of a baseline survey associated with the pressure reduction to hp related to the leak at 1060 knollwood rd (tg) {see PHMSA report 20120033-15480} on the 12" 245 psi transmission main. Gas construction and supervision were dispatched immediately and the leak investigation indicated a probable transmission main leak. An operational plan was developed and upon excavating over the 12" transmission main (at 55 psi) a coupling was found leaking at both compression ends at 22:11 hrs. A welded repair sleeve was installed over the leaking coupling and the repair was tested and passed. Transmission pressure was restored incrementally and on march 8, 2012 at 17:59 transmission pressure was fully restored to 231 psi. Gr-484 was returned to service. The n.Y.S. Psc and u.S. Dot were notified. There were no injuries or evacuations associated with this event. This was a serious level incident.
