Narrative
On 11-18-10 at 12:39 hrs leak survey notified the gas emergency response center (gerc) of a strong odor at 144 bradhurst ave., mt. Pleasant. A code murre was initiated and the mt. Pleasant fire dept. (mpfd), gas distribution services (gds), gas construction and westchester gas operations supervision were dispatched. All buildings in the area checked were found to be free of gas readings (aka- clear) with the assist of mpfd, and subsurface structures in the area were also clear. After investigating over the 12" high pressure pe main and the 12" transmission steel main it was determined the leak was on the transmission main which supplies regulator station gr-484 at brighton and newman aves. Regulator stations gr-519, 673 and 483 were raised to compensate for loss of gr-484. Gds crews were dispatched to v-31585. Gas control, pressure control were notified, and westr general mgr. Thaker & chief transmission engineer payne were advised and responded. V-31585 was closed at 1426 hrs and gas control reported no system impact. V-31587 was closed as a contingency and no system impact resulted. Transmission pressure was reduced to high pressure (60 psi) and the transmission main was excavated over the highest reading. Construction found a leak on a 12" compression coupling and a welded repair sleeve was installed over the coupling. The sleeve was installed on 11-19-10 and at 14:16 hrs mv-31587 was fully opened, at 1459 mv-31585 was fully opened and regulator station gr-484 was placed back in service by pressure control. The US DOT and nys psc were notified. This was a serious level event. Note--this gas distribution incident report is generated to replace rescinded gas transmission incident report 20100100-15165
