On november 12, 2010, at 09:52 the new york city fire deptartment (fdny) reported a contractor damage at 490 hunts point ave., bronx, new york to the gas emergency response center (gerc). The fdny stated that they were evacuating surrounding properties. A code murre (multiple resource response event) was initiated and the gerc dispatched gas distribution services (gds), gas emergency response group (erg), construction emergency response force (erf) and bronx gas operations supervision. A third party fork lift (operated by luis mejia, an employee of hernandez products corporation) hit a transmission pressure gauge post causing a leak at transmission pressure on a 1" steel gauge line. Gds shut main valve 7735 (4" transmission inlet valve to regulator station gr-184) at 10:04 hrs. To secure the leak. The fdny evacuated 10 people from the signature savings bank at 423 hunts point ave. And 6 people from hernandez products corporation at 490 hunts point ave. Prior to gds' arrival, and the evacuation ended at 10:18. Gds, construction and pressure control responded to location. Construction cut and capped the line, welders secured the piping, repairs were made and x-rayed, and the station and effected pipeline were placed back in service at 17:03 hours. This incident had no impact on the transmission or distribution systems. There were no reported injuries, no outages, and no media attention.The US DOT, nys psc and nyc dep were notified. This was an upgraded level incident. Note--this gas distribution incident report was created in place of rescinded gas transmission incident report #20100094-15866.
