Third party arzo inc. Damaged ong's 2" main during a boring operation to install fiber optic cable in a residential area. The third party did request a locate, which was performed timely and accurately. Damage occured as a result of failure to maintain clearance between the boring head and ong's pipeline. Part a, 16. The evacuation value is an estimate provided from the fire department report. Part d, 2a. The estimated cost of public and non-operator private property damage was developed after a review of the fire department report and county assessor records. Part c, 3d. We are unable to determine the date of manufacture, however it is likely between the years of 1969 and 1972. Part c, 5. The puncuture dimensions have been corrected from the original report as 2.0" x 2.0".
