Narrative
Approximately .36 barrels of crude oil was released when the mechanical seal on a booster pump failed. Wear marks on the rotating and stationary seal assemblies indicate the failure may have been caused by an inbalance of the pump impeller possibly caused by foreign matter stuck in the impeller or a faulty radial bearing on the motor driver. The pump, motor and seal were replaced.
