Narrative
Tractor trailer roll-over at 06:00 on sunday june 24, 2012, striking above ground meter causing release and ignition of natural gas and fire damage to the restaurant located at 390 fall river ave, seekonk. Service valve was unable to be accessed due to fire, vehicle and emergency vehicles. Isolation of gas facilities was achieved via alterate valves in distribution system. There was a breakdown in the notification process which resulted in a delayed nrc call being made.
