On friday, june 15, 2012 a washington gas contractor, d.A. Foster, damaged a 1 1/2 inch wrapped steel low pressure service line located at 1120 rhode island ave, nw, dc. At 10:20 am washington gas was notified by the dc fire department of a damaged service line that had ignited. A washington gas crew arrived on site at 10:29 am and made the area safe. The gas was off at the damaged service line by 10:48 am. The washington gas crew removed the service tee from the main and sealed the main with a steel plug. Crew checked structures on each side of affected address for evidence of gas migration. The washington gas crew also performed bar hole testing over the gas main and checked all manholes in area for evidence of migration. None was found. Service replacement was completed on june 28, 2012. As a result of the investigation, the cause of the incident has been determined to be the failure of the crew performing the work to follow washington gas procedure by using a potential spark producing mechanized tool to cut a live gas service.
