A 3/8" stainless steel thermal relief fitting was improperly torqued resulting in the tubing not being properly locked into the compression fitting. The tubing slid out of the fitting and sprayed crude oil on the meter skid containment area. Re-submitted on 3/26/2013 to include part e5f per PHMSA request.
