Narrative
On september 27, 2011 at 1100 hrs, petmar construction laborer, jesus becerra age 49, injured his chin while assigned to a gas main insertion. While personnel on location were attempting to insert 3" low pressure polyethelene (lppe) piping in a 4" low pressure cast iron sleeve, the 3" lppe became stuck. Mr. Becerra entered the excavation and the 3" lppe which was partially inserted, moved and struck mr. Becerra knocking him backwards. The new rochelle fire department was notified and responded. The contractor employee was transported to sound shore hospital via ems. Mr. Bucerra was admitted to the hospital for observation overnight and was released from sound shore hospital the next day, september 28, 2011 at 1640 hrs. The nys public service commision and the US dept. Of transportation, via the national response center, were notified. This was a serious level incident.

The Contractor Employee Entere The Excavation At The Point Of Insertion And While Investigating The Point Of Insertion, The Polyethelene Pipe Shifted And Struck The Contractor Causing An Injury.
