Narrative
A maintenance worker was excavating with a mini-trackhoe when a 1" pe service line was damaged. The maintenance worker attempted to bend over the service and install hose clamps to control the release of gas. The maintenance worker introduced a source of ignition when a battery operated drill was used to tighten the hose clamps. The release of gas ignited resulting in second degree burns to the arm of the maintenance worker necessitating one night of inpatient hospitalization. The 1" pe service was properly located and marked. Supplemental report created to correct locate ticket number.
