Narrative
On friday june 3, 2011 at 1508 hrs the gas emergency response center (gerc) received a call from the fire department of the city of new york (fdny) reporting a gas odor in the area of 75 ave. And main st. The con edison electric emergency control center called the gerc at 1512 hrs. Reporting a manhole fire at the same location with a gas odor present. Gas distribution services (gds), supervision and construction were dispatched. Gds arrived at 1515 hrs. And confirmed the electric manhole was on fire on the northeast corner of 75 ave. And main st. Gds detected a strong gas odor in the area, the gerc & gas control provided the locations of four main valves (20150, 12277, 10099 and 11947) needed to isolate the high pressure distribution system in this intersection. Gds located and closed the four main valves to isolate the area. The fdny evacuated an unknown number of people from 12 buildings (7 commercial and 5 residential) as a precaution. Gas construction excavated three fire banks north, south and east of the fire and inserted stoppers in the 4" steel main on main st. And in the 2" steel main on 75 ave. And verified shut-down. The fdny extinguished the fire in the intersection at 1829 hrs. Three gas services were interrupted; 73-27 main st., 75-03 main st. And 75-09 main st. The fdny and gds re-checked all buildings that were evacuated, all were found clear and the evacuation ended at 1945 hrs. The fdny, nypd, oem, dep hazmat were on location. Gas construction remained on location to make repairs and (2) of the three gas services were restored that same evening at 2230 hrs. The 4" main on main st. Was cut and capped and between 73 ave. And 75 rd., and the 2" steel main on 75 ave. Was cut and capped at the intersection at main st. Gas construction crews installed a new 4" hppe main on main st from 73 ave. To 75 rd. Gas service was tied on main st. Going north/south, and the abandoned, damaged section effected by the burnout was excavated and the cause of the leak was confirmed due to a burnout. Total cost of damage from this event exceeded $50,000. The US DOT, nys psc and nyc dep were notified. This was a serious level incident.
