This release occured when the valve (2750) was opened during the process of making a crude oil delivery. It is thought that during construction acitivities at the tank farm facility a third party contractor broke the drain line on the valve body while performing construction work and when the operator opened the valve to make a delivery it resulted in the release. The release was contained to the ground within the tank farm and properly cleaned up. Supplemental narrative: this supplemental report is submitted at the request of PHMSA to more specifically report the cause of the accident. The cause reported in this supplemental report under g4 more accurately states the cause.

Narrative
