Narrative
A valve that is normally secured open to allow for thermal expansion was closed during maintenance work for isolation purposes. Upon completion of the work the valve was not re-secured in the open position. This resulted in thermal pressure build up in an isolated section of terminal piping and caused a flange gasket to leak.
