Swedgelock fitting was installed incorrectly. 7-18-2011 update - on 10/08/2010 at 05:30, operator discovered a tubing leak on the p62 line at the beaumont terminal. Leak was the result of incorrect tubing installation. The ferrel was installed backwards on the 1/2 inch tubing. Swagelok representatives have been requested to give formal training on tube fitting installation. Employees and contractors are required to use feeler gauge when doing fitting installation. This will be stated in any job plan, if applicable. The enterprise oq committee (which reviews the current task list for gas and liquids) will consider this maintenance activity to be included in the covered task program. The next meeting scheduled for the enterprise oq committee is in the first quarter of 2012 .
