A bleeder valve was left open during a project to remove a check valve at roxana woodpat station. The valve was being used to make sure pressure was not building on the station piping that had been drained to remove a check valve. After valve work was complete and line was being refilled (recommissioned), the leak was discovered. The valve was in the incorrect position for normal operations. Mpl employee did not adhere to the procedure leading to the incorrect operation during maintenance event.
