Performing a 100 pound air pressure test on a 54 foot segment of 20-inch diameter steel gas main when an unexposed 20-inch bolted - type mechanical coupling gave way propelling a section of the 20-inch diameter gas main forward. The test was performed as part of uprating an existing steel main from low to medium pressure. Data entry note: I was unable to enter 1954 for the actual year of install of the pipe in part c, question 2a. The program would not let me enter a year less than 1959. The "unknown" box was checked to allow electronic completion of the form, but the pipe was installed in 1954.
