Narrative
An electric utility was installing a power pole at the location of the incident and augered into a plastic natural gas line causing a release of gas. The gas ignited, and an electric utility employee in a crane bucket above the damage was burned and required hospitalization. There was no request made for a utility mark-out in the excavation area prior to excavation. A supplemental report is required because the electric utility employee hospitalized as a result of this incident passed away due to injuries sustained in the incident. The death occurred after submission of the initial incident report.
