July 16, 2010 office of pipeline safety pipeline and hazardous materials safety admin. U.S. Department of transportation information resources manager, php-10 1200 new jersey avenue, se washington, dc 20590-0001 re: incident #947882 ' 3611 croydon road, henrico, va gentlemen: this is a follow-up to the telephone call placed on july 15, 2010 regarding incident #947882 which occurred at 3611 croydon road in the county of henrico, virginia. The incident, which was reported at 22:24 on thursday 15, 2010, was a fire-first incident and not jurisdictional to the city of richmond, department of public utilities' gas distribution system. This conclusion was reached after confirming that the fire started accidentally in a utility closet more than 15' from the gas meter. The damage was contained to the utility closet and the carport area of the structure. All gas piping was found in tact and gas tight. While the damage may have reached the $50,000 reporting threshold it was determined by mr. Billy garrett of henrico fire department that natural gas was not involved. The city of richmond, department of public utilities, performed a leak survey of the outer perimeter of the building and pressure tested the service line, and no leakage was found. Also at the time, the city of richmond, department of public utilities, had an odorometer test performed by the supervisor of pressure control. This test found the odorant to be in the acceptable range. Based on the above findings, the city of richmond concludes there was no reportable incident. Sincerely, carl v.
