Narrative
On june 23, 2010 at approximately 12:30 p.M. Michel's corporation struck a columbia gas of virginia (cgv) 4" weld treated high pressure (400 psig) pipeline while excavating perpendicular to the facility. As a result, service was lost downstream to four (4) residential and one (1) industrial customer for 37 1/2 hours. Crews worked continuously until repairs were made and service fully restored on 6/25/10 at 4:10 a.M. Upon investigation it is concluded that michel's corporation had a valid one call miss utility of virginia locate ticket #b016000098 for the area of excavation. This locate request was performed by consolidated utility services on 6/11/10. Ultimately locate ticket #b016000098 was cleared in the miss utility of virginia positive response system with a disposition of "10" or "marked". Further investigation revealed that although consolidated utility services cleared or coded ticket #b016000098 it did not mark the entire parameter of the ticket as requested by the excavator. As a result michel's corporation proceeded to excavate within the scope of the ticket, but beyond the bounds of the locate markings placed by consolidated utility services, which ultimately resulted in damage to cgv's pipeline.
